Items needed for Tackle Football Registration

1. Pop Warner Physical Form completed by a physician after 1/1/18 – (this is the ONLY acceptable physical form – school forms are not acceptable). 2018 Pop Warner Physical Form
2. Pop Warner Player Contract & Parental Consent Form. 2018 Pop Warner Player Contract
3. 1  Wallet  sized picture of face (no hats) – The picture must fit in a 2″ x 2″ space.
4. Copy of original birth certificate (with state seal visible).
5. Copy of health insurance card – front & back.
6. One of the following matching your address within the Palm Beach Gardens Boundaries, name on document must match Pop Warner Paperwork:
   1. Copy of Drivers License
   2. Copies of a recent Utility Bill
   3. Government Document with Address
   4. Report card with address matching the consent form
7. 1 copy of  final report card – front & back (must have all 3 semesters/4 quarters) Final, as in last report card of this school year.
